This page gives an example of how to use User
Defined Records to track Diabetes Values (Glucose and
HgbA1c) with the Team Chart Concept
. You already know about the health risks
of diabetes. Your patients may, too. But you may not know all of
your patients' specific Glucose and Hemoglobin A1c levels, and how
those levels may have changed over time.
With the Team
Chart Concept from Ulrich Medical Concepts
, you can track any patient's relevant lab values for diabetes
(or any other illness for that matter). You can then run specific
patient reports, instantly (even when counseling the patient in
your exam room). For example:

The above report displays Glucose and Hemoglobin
A1c values for an actual patient cared for by a physician using
the Team Chart Concept .
All medications that have been prescribed are also displayed. The
Glucose and HgbA1c columns are
User Defined Fields stored in the Team
Chart Concept .
The column Presc->Medication is pulled from
the Patient's Prescriptions.
As helpful as the previous report may be, a graph
of the report can be even more useful. For example:

The above graph displays our previous report.
Glucose values have been scaled to fit on the graph. Notice that
vertical bars appear on the graph to show when the medications Glucotrol
and Lactulose were prescribed. Of course, you can choose which lab
values and medications you want to display on your graphs.
The Team Chart
Concept can track lab values, and medications for
any patient over time. You could also track lifestyle changes a
patient may make (e.g. smoking cessation, increased exercise, etc.)
and display those on your reports and graphs.
Tracking these values allows you to produce very
meaningful graphs. If you desire, you can print these for your patients.
Graphs showing progress made due to medication or lifestyle changes
can be a very effective motivational tool.
