The Team
Chart Concept gives the busy provider (and his/her staff)
the ability to automate the creation of Referral
Letters. Mail-merge type Referral
Letters are initially created as Microsoft Word Documents. These
documents have specific places-holders that you insert to inform the Team
Chart Concept where specific fields should be inserted. A
typical mail-merge type Referral Letter might look something like
this:
Dear [*1],
Thank you
for referring [*2]. I examined [*3] on [*4].
My plan
for her care is
[*5]
Again [*6],
thank you for your referring [*2] to me.
Sincerely,
[*7]
where the following information will be automatically
inserted by the Team Chart Concept into
a specific Referral Letter to be mailed to a specific referring physician:
- [*1] Referring
Physician's Full Name
- [*2] Patient's
Full Name
- [*3] Patient's
First Name
- [*4] Encounter
Date
- [*5] Plan
Category taken from the Attending Provider's Dictation
- [*6] Referring
Physician's First Name
- [*7] Attending
Physician's First Name
Once
the initial document is set up and also defined in the Team
Chart Concept, it is simply a mouse click to automatically generate
a Referral Letter. In fact Referral
Letters can be automatically generated as a side-effect of selecting
pre-defined phrases in your Templates while entering dictation. All
your staff has to do is print (or e-mail) the Referral
Letters (and that can be automated as well).
