Everybody keeps track of Appointments and Encounters.
But often times there are things that your office may need
to track that no one else does, or even if they do, you want to
track those things in your own unique way.
For example, you might need to keep track of
holiday office gifts. To
track holiday gifts, you might want the following fields:
- Patient Name (Giver)
- Date Gift Received
- Holiday
- Gift Description
- Perishable ?
- Estimated Value $
- Thank you card mailed ?
The Team
Chart Concept’s User Defined Fields and Records
are perfectly suited to track information of this form.
Notice that there are seven fields in the above list, the
first two of which the Team Chart Concept
will always supply automatically (Contact and Date).
You have the ability to define the remaining five fields
and you can further specify the type of each field (e.g. text, numeric,
monetary, yes/no, etc.).
In addition to field types such as text, numeric, date, etc., you
can also define computed fields. Computed Fields are assigned
values that are dependent on other field values. For example,
if you have a weight field and a height field, you can define a
computed field to function as a body mass index calculated as weight
field divided by the height field. Then, at the time the operator
enters the weight and height, the body mass index will be computed

Any User Defined Record that is entered on behalf
of a patient is then stored in the Patient's Timeline. It
is treated just like Appointments, Encounters, Prescriptions, etc.
You can find, view, and edit all User Defined Records
in the Patient or System Timeline just as you would any 'standard'
Timeline entry.
Not only can you define and
track your own types of information, but you can also run customized
Reports
on this information. Output from your Reports can be exported
into Microsoft Word Documents.
You can even graph numeric fields that appear on User
Defined Timeline Reports.

Considering again, our gift
record above, we could run a report to display all of the gifts
we have received, but for which we have not yet mailed a ‘thank
you’ card. After
building that list, we could create a personalized Microsoft
Word Document, and
mail out thank you cards or letters to all of the gift givers.
